What to Expect at Your First Visit
Your first visit to Miracle Dental is an important one, as it introduces you to our practice and our friendly staff. During your first visit, we will collect background information such as your medical history, and take the time to get to know Dr. Pham. Below you will find additional information to review before your first visit. To help ensure your first visit goes as smoothly as possible, we also invite you to take a few minutes to explore our website and learn about the dentistry and smile solutions we offer.
Printable Patient Forms
If you are a new patient, please fill out all five of the forms below and bring them with you to your first appointment. Each individual needs a complete packet of five forms.
These forms are fillable from your computer, except for a few areas that require a signature. If you don't have a printer at home, please fill them out, save them, and email them to our office at [email protected]. We will be happy to print them out, and you can sign them when you arrive.
